Information submitted through this form will be used only to register you for support group meetings, send meeting-related communications, and administer the National Lichen Planus Support Group. Registration information is stored using Cloudflare services, and confirmation emails are processed through Resend. We do not sell participant information.

You may request correction or deletion of your registration information by emailing [email protected].

Please protect your privacy by avoiding the disclosure of personal or health information that could identify you or another person. Examples may include full names, addresses, phone numbers, email addresses, dates of birth, photographs, medical-record numbers, employer information, healthcare-provider information, and detailed medical histories.

Participants are expected to respect the privacy of others. Meetings may not be recorded, photographed, screen-captured, posted, or shared outside the group without the express permission of everyone involved. Although the National Lichen Planus Support Group asks all participants to maintain confidentiality, the group cannot guarantee the actions of individual participants. Share only what you are comfortable discussing in a group setting.

Meetings provide peer support and general education. Participation does not create a physician-patient relationship and is not a substitute for diagnosis, treatment, individualized medical advice, or emergency services. For personal medical concerns, contact your healthcare professional. For an emergency, contact local emergency services.
